Take the Time to Be Heard, Your Opinion Counts

Sequoyah Memorial Hospital has partnered with the Oklahoma Office of Rural Health and is requesting your assistance in completing a local health survey as a part of the community health needs assessment. Your input is vital in identifying and solving health concerns in our community and surrounding areas.

The process will also consist of four community meetings. The meeting dates and discussion items are listed below. Each meeting will be held over the lunch hour (12:00-1:00 pm) and will not exceed one hour. All of the meetings will be held at the Sequoyah County Health Department. The first meeting will also provide a more in-depth overview of what will occur during the process.

  • Meeting 1, June 4, 2015: Overview of Community Health Needs Assessment Process, Economic Impact of Health Sector, and Distribute Surveys
  • Meeting 2, June 11, 2015: Discussion of Local Health Indicators and Outcomes and Identify Health Priorities
  • Meeting 3, June 25, 2015: Community Survey Results and Primary Care Physician Demand Analysis Presentation
  • Meeting 4, July 9, 2015: Develop Recommendations of how to Address Health Priorities

Please pick up a survey at Sequoyah Memorial Hospital, and return it to the hospital by June 18. If you would prefer to complete the web-based version of the survey, it can be found at https://www.surveymonkey.com/r/Sallisaw. The survey questions are the same; there are just two different methods to make completion easier. The survey will run until June 18, and the survey results will be presented at the June 25 community meeting.
