Home Health

Skilled nurses, therapists, and certified home health aides provide care and therapy in the patient’s home. Rehabilitative and disease management services also offered.


A non-profit agency, SMH Hospice is a member of the National Hospice and Palliative Care Organization (NHPCO) and the Oklahoma Hospice and Palliative Care Association (OHPCA). Experienced and skilled medical professionals provide end of life care to the terminally ill of all ages. The Hospice team consists of Medical Director Bert N. Corley, M.D., skilled nurses, certified home health aides, therapists, a medical social worker, a chaplain, and volunteers. This team provides care for not only the patient, but also their family during the end of life. Follow up and bereavement care is provided for one year after a loved one passes. Sequoyah Memorial Hospice is also trained and partnered with the Veterans Administration to provide end of life care to veterans.

Pet Therapy

Sequoyah Memorial was also the first agency in our area to provide pet therapy. Molly the Greyhound and her handler, Cami, provide important emotional support and care to our patients.
